Exceptional Family Member Program

(EFMP FMTS/FDI) 

The Exceptional Family Member Program (EFMP) is designed to make sure that the Air Force is able to support your family’s medical and educational needs. It is a mandatory enrollment program for all Active Duty Airmen who have one or more dependents that require special medical or educational support.

The Air Force has developed a new online process for this, but please feel free to reach out to your EFMP office with any questions or concerns.

OCONUS

A Family Member Travel Screening (FMTS) is required when an Active Duty Member (ADM) is seeking an accompanied overseas assignment. To complete the FMTS, please review the guidelines below.

STEP 1 – Register for an account in Q-base (requires a CAC) - https://www.afspecialneeds.af.mil. Once your account is created, access for registration will be granted by your local Special Needs Coordinator.

STEP 2 – Upon registration you will receive and submit all required FMTS documents via Q-base. The EFMP team will update the status to, “In-Review”

STEP 3 – Your family will be scheduled for an Interview with a Medical Review Officer and Special Needs Coordinator. From there, the application can either be locally cleared, or sent to the gaining base as a Facility Determination Inquiry (FDI)

*Please note, A Facility Determination Inquiry is required for all EFMP enrolled family members.

CONUS NON-EFMP

Upon assignment notification (RIP) your Outbound Assignments Counselor will provide an AF 4380 as part of your out-processing check-list. If you do NOT have a dependent enrolled in EFMP and you mark all “No” responses on the AF 4380, then you do not have to out-process with EFMP. Your Outbound Assignments Counselor will update the virtual MPF EFMP status as “Complete”.

*If you mark “Yes” to one of the responses on the AF 4380 a Family Medical Travel Screening is required for the dependent with that has an educational or medical concern. To complete FMTS, please review the guidelines below.

STEP 2 – Upon registration you will receive and submit all required FMTS documents via Q-base. The EFMP team will update the status to, “In-Review”.

STEP 3 – Your family will be scheduled for an Interview with a Medical Review Officer and Special Needs Coordinator. From there, the application can either be locally cleared, or sent to the gaining base as a Facility Determination Inquiry (FDI).

CONUS EFMP

A Family Member Travel Screening and Facility Determination Inquiry is required when an Active Duty Member (ADM) has a family member enrolled in EFMP and is seeking an accompanied stateside (CONUS) assignment. To complete FMTS, please review the guidelines below.

STEP 1 - Register for an account in Q-base (requires a CAC) - https://www.afspecialneeds.af.mil. Once your account is created, access for registration will be granted by your local Special Needs Coordinator.

STEP 2 – Upon registration you will receive and submit all required FMTS documents via Q-base. The EFMP team will update the status to, “In-Review”.

STEP 3 – Your family will be scheduled for an Interview with a Medical Review Officer and Special Needs Coordinator. From there, the application can either be locally cleared, or sent to the gaining base as a Facility Determination Inquiry (FDI).
