The Exceptional Family Member Program (EFMP) is designed to make sure that the Air Force is able to support your family’s medical and educational needs. It is a mandatory enrollment program for all Active-Duty military who have one or more dependents that require special medical or educational support. Effective 08 September 2020, Airmen and Guardians relocating on a Permanent Change of Station (PCS) with dependents are required to complete the TSQ via MyVector. This online questionnaire replaces the requirement to complete an AF Form 4380, Air Force Special Needs Screener
Effective 30 August 2021, the FMTS process was fully automated via MyVector. Airmen and Guardians with dependents are required to complete the TSQ in conjunction with PCS processing via MyVector. Members will then transition into the FMTS application for one of the following reasons:
• EFMP-enrolled family members (Q-coded Sponsors)
• Family members traveling to an OCONUS assignment
• Answered “yes” to travel screener questions and traveling CONUS
Airmen and Guardians who have dependents will receive a MyVector system-generated email eight (8) days after the Assignment Creation Date (ACD) and are within eight (8) months of their Report-No-Later-Than Date (RNLTD).
CONUS Assignments
• If the member is relocating to a CONUS location, answers “No” to all questions, and has no family members enrolled in EFMP, then the member is cleared to receive PCS orders
• If a member is enrolled in EFMP and answers “Yes” to one of the questions, the member will receive two (2) notifications: the “Additional Screening Required” and the requirement to complete the FMTS application.
• The member will now transition to complete the FMTS application
OCONUS Assignments
• If the member is relocating to an OCONUS location and his/her dependent(s) will accompany them, then the member will receive two (2) notifications: 1) the “Additional Screening Required” and 2) the requirement to complete the FMTS application.
• All dependents accompanying sponsors to an OCONUS location regardless of EFMP enrollment are required to complete the FMTS application.
